Good Morning,
I am trying to formalize our customer satisfaction rating. We are currently ISO 22000 certified. Does anyone have a letter which they send out to their customers to get feed back on how they are doing? Much appreciated.

Hi Janice, I doubt if it's compliant with ISO 22000, but I've attached a Customer Satisfaction Measurement System for ideas.

A sample Customer Feedback Form for a food company.

You can associate rating with each remarks such as

3: for Excellent
2: for Good
1: for Average

to statistically evaluate the customer satisfaction level.
