Has anyone got any example of Certificate of Disposal and Authority to Dispose? For example, disposal of wastages, rejects, or selling of scraps.
I would very much appreciate it.

Dear Mind over matter,



you should get this certificate from your waste contracter. It is a statement that the waste you supplied is destructed and will not enter the food chain again.

Do you mean from Junkshop Representative (external)?

Dear Mind over Matter,

Your waste and rejects has to be collected and handled by licensed/authorised waste collectors/handlers. They should be approved/licensed by the authorities.

For some category of wastes you should be authorised your self. e.g. chemical waste, organic waste (sometimes)

Besides that, you want to make sure, that your rejects, will not end in the food chain again. Your waste collector has to be trustworth and you need to receive a document indicating the amount of waste collected, the sort of waste and the destination of the waste (destruction?)

I do not understand what you mean with junkshop? Is it a second hand store?

Thank you for further explanation, 'Madam A. D-tor. I understand now. Ideally it should be from the 3rd party but our worry is that they might not be able to create a decent one. Perhaps we would create a template and contractor will just fill it up. Yes, it’s a second hand/third hand store. We (people in the Philippines) call them Junkshops.

We will also take into consideration that the handler should have a license or authorization from local govt. Again, thank you for your inputs.
