Yep, and all of them were employee practices based.
I put together a root cause analysis and corrective action plan, but it was turned down by executive management. Though employees have been trained, retrained, coached, and all that, they still managed to do some interesting things in the name of "helping".
I suggested that all management be issued tasers for "immediate corrective action", but the idea was rejected.
Sad.

That sounds tempting, lol. We also have some "creative" employees. They even found a way to get the automatic faucet control box so full of water that it would not work. Twice.

Discovered today that BRC trumps Tesco PIU! Unannounced BRC yesterday & Tesco PIU turned up today, and we had turn him away as we were on day 2!!!
