I'm looking for a sample procedure for returning equipment into process after repairs etc.

Hello,

 

Just follow what you are doing now, but then don't forget to include what the standard requirements to be followed. Ex: it must be verified by an authorized person that it is functional and can do its intended use, no missing parts (if it is done inside prod area). Then it must received by the end user from the maintenance department also must be verified.

Simple: maintenance signs off repair is completed. Sanitation signs off it is clean and ready for use.

 

Extra: Maintenance signs off that repair is completed, sanitation signs off it is clean and ready for use, maintenance observes the equipment once more after installation to verify no broken/missing parts or poor assembly.

don't forget to ensure the pre op is completed prior to use as well

One more additional item: signoff that all tools have been removed. 

We've combined this all into one form... description of problem, equipment involved, circle Y/N for tools removed, area cleaned/sanitized, and pre-op inspection completed, then signoff by supervisor(s) and/or maintenance.

My company uses the attached "Equipment Re-Instatement Checklist" for this purpose...hope it helps!
