Section 1.1.10 of Issue 6 requires a root cause analysis of non-conformities identified in the previous certification audit. If the previous audit was to Issue 5, then does that mean a documented root cause analysis is not required this time around?

Also, does anyone have some simple guidance on the requirements of root cause analysis?

Unfortunately, yes it does. The BRC confirmed this specifically in their Frequently Asked Questions document.

I think that these FAQs have been posted previously, but here they are again for convenience.

The root cause analysis system that most people seem to be using is the '5 whys' model.

I am not sure that I am convinced of the real value of such a deep root cause system. In a lot of cases it is just common sense and the danger is that you do it ritualistically just to please an auditor.

Ultimately, most root causes come down to money and people.

I believe that BRC is contradicting themselves. In the BRC Interpretation Guideline for Issue 6 it reads, "Non-conformities identified in previous certification audits shall have been fully and effectively rectified and these will be checked during the current audit. Where the previous audit was carried out against Issue 6, the effective identification and correction of the root cause should prevent recurrence of the exact or similar non-conformities."
