hi everyone my I'm M. Ortega and I have join to learn more about the food industry, at the moment my goal is to learn more about root cause.

Hi M. Ortega and welcome.

Root cause is generally the origin reason for something. Is your query about root cause in general, or do you have a specific case you need to investigate?

As a broad example, a catering customer may complain that they received the wrong product from the manufacuter. An investigation finds that a new member of the packing staff packed the affected case. When asked, the new member of staff had not been shown properly how to find and follow the packing specifications and packed the wrong product by mistake. The root cause in this case would be a lack of proper training to the staff member. You could also look at why it wasn't detected in dispatch QA checks, why the training wasn't done properly etc etc but this is a broad example. The exact root cause will always depend on the situation and factors involved, and often there will be a number of factors connected. Root cause can then pave the way for corrective actions aimed to prevent re-occurence.

Honestly, root cause is very complex, because after you carry out root causes and say you found 5 root causes of your problems, there is prioritization matrix that you have to do to really determine what issues/ problems to address.

 

The normal root cause analysis workshops that I've attended over the years only pointed out methods to use to identify the root causes like 5 whys, fish bone diagram etc etc. but not how to deal with the actual identified root causes. I suggest taking a lean management workshop towards yellow belt, that's where i learned the prioritization matrix and the other good stuff.
