Short answer: No.

 

Long answer:  You need to determine your site specifics, using whatever standards you are being held accountable to (third-party certifications, government regulations, etc).  Every facility is going to have a different policy, because it's not supposed to be a "cookie cutter" policy in any way.

And if anyone is not aware, FDA finally issued the final part of their Guidance documents for the IA Rule.
