Does anyone have any sample forms for conducting a mock recall?

Does anyone have any sample forms for conducting a mock recall? The forms provided by our consultant are a disaster, and I'm on a hunt for a better template. 

 

Forms should be applicable for a traceability within a baked goods company.

Thank you! Looks like the more appropriate terminology for what I needed was a "traceability exercise" template (terminology seems to be half the battle. What you sent me was a great starting point, and I will be able to put together a much more effective form for future exercises.

Great Help.... My knowledge about mock recall as per this reference document was only practical, I am doing all activities of four components of Recall/mock recall excercise. But this information absent in our SOP. I am going to two excercise right now.
1. Preparing PPT for training purposes
2. Revising my SOP.
