Struggling with root cause analysis for two minor audit non-conformances

Good afternoon :)

first of all Many many many thanks for all brilliant help that I always found here!

I passed my BRC AUDIT 6 days ago with 2 minors and I am struggle now to find root cause analysis.

 

4.9.1.1

Two containers of un-labelled liquid observed in chemical store

 

 

4.11.8.3

The circumstances under which a review of the environmental monitoring risk assessment have not been documented

 

 

Would anyone like to give some nice advises?

Hi Serena.Sorbello,

 

Congrats on your audit!

A usefull trick is to ask yourself ''why'' about six times (or at least one more time after you have grown annoyed by it). If you aks yourself ''why'' enough you will eventually get to the root cause.

Example:
1) Why was the container of un-labelled liquid in the chemical storage? -> Because the wrong container was orderd by planning and this container did not contain a label.
2) Why was the wrong container orderd (and why wasn't it noticed)? -> Because planning was unaware about the need for correct containers. It was not noticed because quality/production does not perform any checks for it.
3) Why was planning not aware of the need for correct labelling and why are there no checks implemented? ->........

It's all about context. :smile:

 

First one was lucky to only be a Minor IMO.

First ? is  - How far back does the problem Go ?

I suggest you check out the BRC Understanding root cause analysis 06/01/2012. I believe it's still available on the BRCGS website.

Perhaps depending on what it actually is and how it can interact with other chemicals.

Referred to as the "5 Whys" but if it's a less complicated issue you only need ask "why" as many times as necessary to reach root cause. While if more complicated maybe 6 or 7 but usually no more than 5.

Hi Guys,

thank you very much for all your answers!

I do know the 5 whys technique :)

I was asking any suggestion how to approach on this two cases.

it was my first BRC audit, I do not have such a big experience and I look after all quality system by myself ( due a very small company, only 7 people and only one product) so for me only two minors was a great result!  :spoton:

I will investigate about the unlabelled chemical in the cupboard and for environmental monitoring it was just an overlooked part of the standard.

 

I find this forum a very precious help for me.
