Hi carine; I would add potential hazards (e.g. biological, like TC, E. Coli, etc., chemical like metals, etc., physical like rust, etc.)

Hi Carine

Depending on the nature of your raw materials:

For example you may handle concentrated flavours, or high strength powder, for instance ascorbic acid, and in those cases health & safety information might be useful?.... unless you have separate MSDS for each ingredient?....

The packaging format might be useful too, unless this information is held on a formal specification (agreed between yourself & the supplier?).

The document you have shown appears to be something that would be used to check the quality/packaging of incoming raw materials rather than something you would agree with a supplier (like a formal specification).

As a minimum I think we need to address the points mentioned in clause 8.5.1.2
a) biological, chemical and physical characteristics;
b) composition of formulated ingredients, including additives and processing aids;
c) source (e.g. animal, mineral or vegetable);
d) place of origin (provenance);
e) method of production;
f) method of packaging and delivery;
g) storage conditions and shelf life;
h) preparation and/or handling before use or processing;
i) acceptance criteria related to food safety or specifications of purchased materials and ingredients
appropriate to their intended use.

This is the minimum requirement and you need to add product specific requirements are well, these can be found in local mandatory and legal requirements, microbiology, physical and chemical limits applicable.
