Hello and Good Wishes for the new year. Trust you are all well and keeping safe. (wearing your mask and sanitising). I need some assistance regarding improving on my customer Satisfaction Survey Questionnaire. Below is what I am currently using however I was asked to improve and benchmark against other food company's questionnaire.
 

"Our company, in line with our Food Safety & Quality Management System, aims to constantly improve on its services to you our valuable Customer.   We would be grateful if you could spare a few minutes to complete this Customer Satisfaction Questionnaire to help ensure that our standard of customer care exceeds expectations wherever possible.

 

Please tick the appropriate box to indicate your degree of satisfaction, where:

1 = Excellent,    2 = Good,     3 = Satisfactory,    4 = Poor
