What needs to be on a Certificate of Conformance for food contact packaging?

What exactly needs to be on a COC for food contact packaging ie - bags, rollstock etc .? Are these items mandatory? I have COC's that are very limited in information. Approved for Food Contact Compliance with the Federal Food, Drug & Cosemtic Act Resin levels  Allergens. Thanks in advance!

PS - I think the above is a relatively "good" COC, IMEX some examples are considerably less detailed.
