Does any body has  sample document for Risk Bases environmental  monitoring programme as per the fssc additional requirement 2.5.7

I could not find my template that I used in the past, but just use a standard hazard analysis table (i.e. (1) step, (2) identify potential hazards,  (3) risk assessment ratings, (4) justify your decision, (5) measures apply to control hazards, (6) is this step a CCP/ CP?

This will be easy and sufficient to conduct your risk assessment.
