Approved Supplier Declaration for Service Providers

Hi all,

 

Can anybody share what Approved Supplier Declaration they use for service providers. i.e. pest control, fork lift servicing, transport companies, cleaners etc.

 

Currently I just get their insurances, however auditors question why I don't get supplier declarations.

 

Our supplier declaration is way too involved for service providers. I'm at a loss of what I need to be asking them.

 

Any help would be much appreciated.

Standard declarations and questionnaires don't work for service providers as generally they provide a specific function.

 

Look at what they do for you and draw up something fairly loose and open.

 

In your examples, most pest controllers give you their service agreement such as 8 visits,4 Field Biologist inspections call outs, follow ups etc. If you are happy with that just get both parties to sign it.

 

In the case of Fork Lift Servicing, something like the 'service provider will visit and provide preventative maintenance on 10 fork lift trucks 4 times annually and will attend for breakdowns and unplanned repairs as requested by the company. The cost for these services will be agreed by the client and the service provider on an ongoing basis'

 

Always put in a clause that those attending the site will follow the site rules and will advise the client if there are any risks to personnel and materials from the work that they are doing.

 

I also like to do a summary of the various service providers, what they do and how you approve them. I will post it later as I will need to hide some identities

This is the summary.

 

It has an annual review and helps to keep on top of who is being used.

 

Its downside is that it makes an easy checklist for auditors to find somebody you have missed off.

One thing that few people think about and I have only been asked once by an auditor is with a service provider like a PCO that will be working in your production areas is allergen control and cross contact. Since the PCO typically doesn't come in contact with zone 1 equipment this is not really a high threat to your process but we had to train our PCOs to notify us if they need to work within 5 feet of exposed product or equipment that they will need to wear a company smock.
