Not that it matters, in the larger scope of things - but can I ask which GFSI Scheme you are under?

 

Also, how are you anticipating using this survey? ( who is questioning who?)

 

Typically an inclusive survey will include the following for a specific frame of time (e.g. quarterly):

Review of pest control measures ( Your question #6)

Review of Bait takes (for outside control)

Catch analysis from trapping devices (Maybe your question #1)

Identification of any trends (This will point out where your PCO could be doing a better job or where they are doing a good job) 

Review of PCO's actions  (Did you have to fog? Use new chemical? Add more traps?)

Any recommendations from the PCO for changes or improvements.

I think it is fine if you just want to specifically look at pests that are or might be in your facility.

I think a "Pest Control Survey" should take into account the building fabric, pest control devices, administration of the program, etc.

You might want to consider adding those items. See attached example.

Just few a suggestions, It should mention the area (External/ Internal)

 

and also,  Corrective Actions!?

 

 

Is your pest control  is outsourced?

 

If yes, is it approved? You can ask them to provide Pest Control Survey report.

Hi In my opinion, a form like this is really asking about failures in pest control service rather than asking about the quality of that service. Mgourley's form is a more standard audit style form which would be used when carrying out a full audit of the pest control company's performance. If this is for use by your own staff, then there needs to be some account taken of the likelihood that they will see pests at all (I dont know what sort of plant you have, but not all operators will have an equal chance of seeing pests). There should also be some form of pest awareness training going on - perhaps a useful question would relate to that? Why would an operator be given details of what the pest controller has done, including a detailed ID? Surely that would be for discussion with the site contact/contract manager/QA team? Assessing the quality of your pest control provision is hugely important, but may I respectfully suggest you think this through a bit further and make sure that you are asking the correct questions of the correct people? Best wishes
