Does anyone have a concise internal audit report form.  I'm looking for a one pager, no more than two.  Thanks.

The most common generic solution is to use a version of the Standard involved. Not exactly concise though.

As Charles says, it will depend entirely on the Standard you are using.

Attached is a paper form that I have used.

Auditor fills in the associated Policies/Procedures/Records (and of course audits them)

Then there is the actual Standard clauses

Then there is some specific prompting to the auditor

I work with John and we are seeking SQF certification for food contact packaging.  We are trying to firm up our Internal Audit schedule, plan and training.  We were looking for a straightforward document that our internal auditors can use, can easily understand, but will still meet all the requirements for the standard.

The way that we have ours set up is I have each specific section of the code per sheet.

 

So say, for Waste Disposal - my form has the date, time, those present for the audit (myself and if I have someone from another department), then the requirements for 11.9.1.1 to 11.9.1.6 (6 lines) on the sheet.  

 

That way, I have that specific sheet for that specific audit each time the audit is completed.  I have audits I do monthly, quarterly, semi-annually, and annually depending on risk.
