Nicola : Do the US have automated packaging systems on line to ensure the right packaging is used. In UK we used 2 d barcodes to check version of packaging. This does not stop someone putting wrong product into right packaging though

Brian : We process oats, which will sometimes contain adventitious wheat and/or soy.  FDA notes from a few years ago state that this specific kind of inadvertent cross-contamination does NOT need to be declared.  Has this changed under FSMA, in terms of risk-based hazard analysis?

Kris : Root cause analysis can be very time consuming and therefore not always supported by all parties within the company. How do "sell" its importance, build it into the company culture? Would be keen to hear your thoughts, advice on this please?

Rachel : Does this problem happen with other operators?  Is the problem with a maintenance issue?  I'm not a mechanic but sometimes they don't hold air pressure needed to hold steady (simple fix sometimes).
Tara : Maria--ask yourself this: Did the forklift drop the pallet, or, did the Operator of the forklift drop the pallet? Based on your answer, you would then be able to do further analysis of the root cause
