We have a shear mill / screen set up in our process for filling a paste product into pails. One of our preventive controls is to check the screen for foreign material. Currently we are not documenting correctly if something is found on the screen. Does anyone have a template for tracking this?

Sure - We don't document at all :/ . On our batch ticket there is a sign off that the guys "inspect the screen for integrity and foreign material" and they will initial here, but we do not document any "findings" . So for instance, if an employee were to find a piece of plastic or a other foreign material , he brings it to the supervisor but this is not recorded anywhere. We need a form to be able to record this. 

 

The screen we use is a Process Preventive Control for us so it needs to have a monitoring, corrective action, etc tied to it. So I am not exactly sure what the form would need to entail when we would find a piece of foreign material.

What the material was (photo if you can)

Corrective Actions

Preventative Measures

 

Then input them into a spreadsheet so you can see if there is a trend, include the vendor of the raw material----you may find a particular vendor/ingredient is the suspect generating the FM

We use a non-conformance report which covers a multitude of plant wide issues and is accessible to all personnel
