Hello, I am currently working on implementing an approved supplier program at my company. We are a plastic packaging manufacturer and don't deal with any organic matter, so our suppliers are things like film, polymers, resin, and inks. I'm trying to keep our supplier self evaluation form as simple as possible. I took a sample someone else posted here (I'm sorry I don't remember who!) and have modified it to fit our needs. I'm trying to keep it concise as I know these are a pain for many people to fill, but wanted to get some feedback from the kind people here as to whether there were any unnecessary questions, or additional questions I should add.
