Supplier Self Evaluation Form Feedback?

Hello, I am currently working on implementing an approved supplier program at my company. We are a plastic packaging manufacturer and don't deal with any organic matter, so our suppliers are things like film, polymers, resin, and inks. I'm trying to keep our supplier self evaluation form as simple as possible. I took a sample someone else posted here (I'm sorry I don't remember who!) and have modified it to fit our needs. I'm trying to keep it concise as I know these are a pain for many people to fill, but wanted to get some feedback from the kind people here as to whether there were any unnecessary questions, or additional questions I should add. Thank you for any feedback!

You said that your company is a plastic packaging manufacturer, however, the evaluation is for a food manufacturer whose suppliers are raw materials for input into the finished product.  You need to determine the criteria you will use to evaluate your suppliers of inks, resins, polymers and films. I don't believe questions pertaining to GMP, etc. is suitable here.  Questions more related to the ISO 9001 standard may be used or some other relevant standard.

Ahhh, that is for food and not packaging.

Also, are you planning on auditing these suppliers or getting an auditing company to do it?

Are policies in place for the control of glass, wood, and metal, and exclusion of said materials from production areas?       

 

 

A lot of these questions were based on those from the Supplier Self Assessment form provided through their Food Packaging Safety System, can anyone advise what would be more appropriate questions to ask? 

 

If suppliers are unable to provide audit reports, I figured the need to audit them myself or have someone audit them would be dependent on how high risk they are determined to be. The code doesn't seem to require audits, or am I mistaken in this?

I deduce this relates to 2.3.4.2

 

There are likely some specific offerings on the Forum if you search.

 

The SQF Code seems quite specific in its requirements. And lengthy.

 

I suggest you have a look at the SQF Guidance material for previous version to current since afaik latter is not yet available.

 

 

You can compare yr document with the one in post linked below  (afaik not specifically designed for SQF but it is  "risk-based") -
