I am new to this forum. I plan to conduct supplier audit for primary and secondary packaging used.
Primary packaging is food contact LDPE roll. Secondary packaging is RSC and die cut carton.
Do anyone of you have packaging audit checklist or questionnaires? I need guidelines to establish audit checklist or qeustionnaire.

As Simon has advised the BRC standard is good.
But also look at the "AIB Consolidated Standards for Inspection" - "Food Contact Packaging" and "Nonfood Contact Packaging".
www.aibonline.org Both the BRC and AIB checklists are very good, but you may also need to add your own requirements for Food Safety Compliance documents (FDA/EU etc), for all the raw materials used to make the packaging item, and in the case of flexible film, the final product.
Also check GMP,
HACCP, Pathogen Testing/Management, etc are included. In New Zealand we also refer to the NZFSA Model Pathogen Management Programme -

Primary / Food Contact Packaging, should be made and packed in a Hygiene Room / area. This may not always be the case unless the vendor has recently rebuilt their plant or moved to a new green fields site, and built in a Hygiene room / Redline controls as outlined in a Pathogen Management programme.
There maybe something similar in your own country regs etc, but you can use this model from the NZFSA web site and taylor to your own requirements.
