Foreign Travel Form

Hi,

At the beginning I would like to day how much I like this website which helps me in my day job.

I am now in need of some favour.

I am updating at the moment holidays forms, visitors quesionnaire etc and need to put in place a foreign travel form which people would fill in after the holidays outside the EU.

Can anyone has a copy which would like to share with me please?
