Your opinions on MAN as Root Cause and 'No Blame Policy'

Dear Members,

 

What is your personal opinion about tagging causes to MAN while doing root cause analysis under 'No blame policy'. If I am going to follow a 'no blame policy', I should tag all the causes to either 'Machine', 'Material', 'Method', ........ but not 'Man'.

 

Hope not inducing confusions about traditional 4M Concept ????? :shutup:

Hello Zeeshan,

In a robust and joined up root cause analysis process MAN is very much a valid cause, even under a No Blame Policy.  However, it is another MAN who is responsible for solving the issue MANagement.
