Does anyone have a template for Foreign material contamination incident reporting?

We usually go with a one page of "Date/Hour/Place where the foreign matter was found/Investigation/Corrective action/and paste it together.

Here's one I developed (well, modified from an existing template I found somewhere)...still not sure if we're going to use it or not, may just go with a CAPA form:
