Does anyone have an on the job training template for training sign off?

I was wondering if anyone happened to have an On the Job Training template? Basically what I'm looking for is a document that we can have in our facility that will serve as a training sign-off each time an employee is trained on a particular job function (not including our refresher training).

Broadly speaking, you want a sign off that demonstrates that they have retained some of the knowledge. So a test with some basic questions that they sign and date.

I have attached what we use, I think this is what you are looking for. For some/most training topics we do have a quiz that is separate but they always fill out this training form as well. I will populate the training topic section and supporting documents then print to hand out to employees during training.

I have attached an OJT form I have used in a couple different environments. Mine is a little different than others. It is an exact reflection of the "Job Description" for that employee. In this way, every responsibility identified for that position is on the OJT form. The mechanism being that at a predetermined interval (30 days 90 days) the employee is presented with the OJT form and asked to sign off that they have received sufficient OJT training for their position, at which point they are identified as having received sufficient training by signing off on the OJT form and the supervisor signing off that their observation supports the receipt of sufficient training as well. 
 
Any way, it may not be exactly what you are looking for but I have used it multiple times in audits in conjunction with that employees new hire training and identified refresher training to present an overall training program.

thanks for the attachments - we use ojt for most of the plant work
