Absent BRC/IOP Acc, Self assessment questionnaire required

Do any of you guys have a self assessment questionnaire for my secondary packaging/ cardboard supplier. I have only ever purchased off suppliers with BRC/IOP. I have not got time to audit this company and they have not got any other accreditation.
