Need to Produce a HACCP Plan and CCP Decision Tree

I'm badly tangled up in a case study, to be prepared under CCP determination tree and generic guideline.Could anyone provide me with that?

We are required to come up with a generic HACCP plan for any sort of product and need to produce a CCP determination tree as well.What is that CCP determination tree precisely?Is there any specific tree or are there any specific steps involved in CCP determination?
