Thks for the nice form, ie detailed but not over-revealing.
An auditor (but not the Engineer!) might be disappointed by the absence of the popular phrase "root cause".
I assume "Supervisor" refers to someone other than Engg, eg Production, however it seems rather optimistic that the latter would be competent to generically certify that the work was "completed correctly".?
Strictly speaking, perhaps "verification" should be "validation"?
IMHO, the word "subsequently" in early paragraph might be usefully deletable. KISS.

Please can anybody help. Has anybody got a template document for job request for maintainance to fix.........
There will be a variation of what is required depending on the sensitivity of the equipment being operated on. E.g. Food Contact Surfaces are high risk.
You might want to add some of these checks to the forms that WPC and Zeeshan have posted:
Following maintenance operations there should be checks to confirm:
Shavings and swarf have been removed
Weld debris has been removed
Repair materials (nuts, bolts, wire, screws, seal etc) have been accounted for and removed
Excessive lubricant has been wiped off
Tools and parts have been accounted for
Cleaning materials have been removed
The area has been left clean and tidy
The equipment is cleaned and has been disinfected to the necessary pre-production standard
There is a formal sign off releasing the equipment back into production by an authorised person

We use similar approach and similar forms. However, how do you get around ensuring that people who sign area back to production actually go and check that the area is good to return to production.
How do we ensure that people don't just sign on the paper for signing sake?
ANy comments on how you make this work effectively in your factories. Esp when a contractor has completed work and there is no Produciton supervisor around at the time to sign off.

We use a similar form, but production and QC people sign before the work is done (preventive measures to be taken during production), and when the job is finished QC sign it again to verify the place is clean and free of tools or debris.
