Supplier Audit Questionnaire for Food & Non Food Packaging

Hi.

I was wondering if anybody has any documentation that they can attach to help with designing a new Supplier Audit Questionnaire for Food & Non Food Contact packaging as the current one we have is not good enough in my opinion as there is no questions asked to give enough to approve current suppliers.

I would appreciate it if somebodu out there could help me......

I have attached the one we created to use for our suppliers. We only manufacturing Direct Contact Food Packaging, so you may need to add or change some things to take care of the Food side of things.
