Looking for a Declaration of Compliance Template

Hi Everyone,

 

I've just started working on this field and I need to hit the ground running. I deal with packaging material for the food industry and am in urgernt need of getting my hands on a template for the Declaration of compliance according the newest legislation EU 10 2011. Searched for it allover the place, unsuccessfuly.

 

Would someone be so kind to give me a hand?
