I was just wondering if anyone has a glass breakage register template that I could use.

4.9.3.2, as I read it, requires you to have a register AND a monitoring activity.
The register requires you to list the item, detailing location, number, type & condition
The monitoring activity is a recorded check of condition of items, carried out at specified frequency that is based on the level of risk to the product.
That's how I've interpreted it, hence a separate register and audit document.
