helo . may someone please give me an example of a good pest sighting log sheet and an internal appointment letter for a pest prevention officer

I have a Customer Pest Sighting Log attached. I am not sure what you mean by an appointment letter, though.

I appreciate the value of trend data but the info. seems almost too informative in some respects. Or is this form only for internal use.?

 

Hopefully the entries for major enemies are relatively infrequent (eg Nil) or the data could perhaps self-generate an auditorial NC ?

 

IMEX individual pest control reports tend to diffuse / mitigate reportings of minor sightings such as 5-legged flour hoppers although rodents are a rather different story of course.

Depending on your standard, I think it's an NC to not use a form like this?  Sometimes you see pests during the week and the pest control dude has to know about it, then help you fix whatever is causing it.  This one is pretty boiler plate - mine looks the same  & the few audits I've had they always look at it and say "Good, you're using the sighting log & responding".

There's another form in my book for the pest control tech to write down NCs (door gaps etc) and we complete the corrective actions & I sign off that the problem is fixed

 

per AIB 4.9.1, pest control log needs to have date, time pest sighted, actions taken,  name of person reporting, date, time.

Yes, this is primarily for internal use.  I have not put anything more than insect activity on this sheet (thankfully) Our process is not conducive to rodent infestations. Freezer temps, cold room storages and precooked ingredients. 

 

Our door seals, etc are checked during monthly GMP walk throughs.  I am glad people are finding this helpful!  :biggrin:
