In my experience a just a CAP report is sufficient
1. Description of the non-conformity to be addressed
2. The root of the non-conformity
3. Action to be taken to address the root cause and prevent recurrence of the non-conformity.
4. The responsible person(s)
5. Timeline
6. Activities that the establishment will take to ensure that the action taken addressed and corrected the root cause (Verification)
7. A requirement for record keeping
