Supplier Approval of Contract Service Providers
Hello All,
I am struggling with how to approve contract services that are provided via verbal contract only. How do others handle this?
I'm not sure if I should send them questionnaires and what documentation to request as part of our Supplier Approval Program.

I agree. You need to have some sort of documentation outlining the service they are providing and any restrictions they may have (i.e. not entering the plant, adhering to GMPs, etc.). This should be simple enough for one to mock-up and get both parties to sign.
Make up a code of practice for them to sign , something simple not full of jargon, what you expect from them, what your Hygiene rules are etc.. I have attached a template
I have decided to make a list of "approved contract service providers" listing all of our providers and the service that they provide including contact information. All of our visitors that enter the plant are required to sign a copy of our visitor policy indicating the rules that they must follow while on the premises.

You can simply get a statement on a letterhead from the supplier as to what they provide, etc.
An example of this - We have a client with about 40 plumbers, electricians, silversmiths, chimney sweeps, etc on verbals - we had each one produce a letter on their letterhead indicating what they do for the company, how they are contacted, etc. Each one attended a training on GMPs for contractors and each is required to provide training information, ID's etc and obey the requirements for contractors that each one was sent and they signed and returned.
