GMP tracking and trending program?

Does anyone have a GMP tracking and trending program that they would share? We are looking for ways to address repeat deficiencies - either by the employee or the department. What has been effective in your plant?

For trends I tend to score audits and from this you can compare areas and show improvement.
You can log issues/corrective action in an Excel spreadsheet.
For repeat offenses, I tend to follow up on previous non-conformances in audits even if they have been closed out. Repetitive problems are dealt with by refresher training, mentoring or as a last resort disciplinary action.
