How to address/ analyse root cause analysis for the presence of foreign paticles in bottled drinking water.

Already i knew the cause of the issue it is due to the damaged seal in side the filler.

Use the "5 why's" method.  Why is the material there? Damaged seal, Why is seal damaged?  etc.  You will eventually find the true root cause.
