can someone give me a template for a 3x3 risk assessment matrix used for haccp and definition for severity and likelihood

likelihood:

the state or fact of something's being likely or proper simply the chance of its presence .

Severity:
The seriousness of the effect(s) of a hazard.

 

For example , you can think like : the likelihood of presence of stone in apricot is high ,the severity of it is high since it will cause a serious injury so 3*3=9

and so on ..
