What methods do you use for Root Cause Analysis?

We are now working to issue 5 which heavily features Root Cause Analysis for audits and incidents. I'm just wondering how others are approaching this? What methods do you carry out RCA with?

I've used 5 Whys in a micro lab. Worked well there.

We started using 5 why last year.

Fishbone was used before and still applies sometimes

Depends on what the application is. I use 5-Why for audit or systems non-conformances but I find Fishbone to be most effective for factory floor incidents. Fishbone is especially helpful when engaging stakeholders from a range of departments/skill sets in a group so that the exercise is more comprehensive and adds more value through engagement and ownership of the issues and resolutions.
