Hello qualityfishgirl11,
I'm not sure if this is for SQF, but i have attached my straightforward label review form. Mine is simple and it does the job. I have not gotten any problems by using this form. I attach a label onto the paper and on the side i list the ingredients it contains or attach the batch sheets we use and compare it against the label.
