How to do a Risk Assessment Matrix from getting the Likelihood and Severity?

I was wondering how they come up with that data shows in the chart.

Is it from the Ingredients usage or in Generalized Finished product or from the process of their product?

You can start with the risk assessment by quantifying the severity and the likelihood. A scale from 1-5 is a workable option, as it gives you more nuance than a 1-3 scale.
