New Equipment Comissioning Form

Hello, I am hoping someone has a template they can share as an example? We are a small, simple, plant and all of the sample templates I see on line are super high level, and my team is having difficulty with them.  I am hoping there is an example with just the bare necessities in circulation out in this forum :)   any help or ideas what the minimum requirements/questions should be? (I know it is all risk based and specific to each facility, hoping for a starting point to build off of) Thank you~
