Hazard risk assessment matrix (likelihood/ severity)

Dear All,

 

I am working on hazard analysis of fresh vegetable processing line. Hazard analysis record sheet required for the raw material, ingredients and as well as in process steps.

 

Hazard risk assessment matrix is base of likelihood/ severity that support to hazard is substantial or not substantial.
