Any have template for HACCP plan risk assessment is complete severity and likelihood identification prior to go through decision tree.

Hi Ryan,

 

Thks for template, it's a nice format but seems to imply you have to repeat the whole layout for every identified hazard ? (unless yr Process Steps run along the x-axis ?)

 

PS - the matrix "consequence axis" seems a sort of mixture of safety/quality.

The risk assessment can be used in many ways. This is a good template to use for each process step. Just like you said it would be repeated for each process that you have identified a hazard at. If you want to add it into your hazard analysis, then thats fine too.

 

To me this satisfies auditors better and it also forces whoever is maintaining the HACCP Plan to truly assess each hazard and determine its severity. It more or less proves you did your homework and its all in one place. Depending on how long your process is, that might or might not be the best route to go. Really depends on how much information you want to provide. 

 

The consequence axis can be adjusted for anyone's liking. Its meant to be food safety related, but i have seen different verbiage than the one i presented.

We've been using the template used by Mr. Ryan as well since we have started way back 15 years ago. We found it so easy and convenient to the team

during risk assessment.

Is this template used along side a standard hazard analysis or as a stand alone?
