Chemical Request Form

I was tasked by my Quality Manager with revising a previously lengthy chemical request form.  They want it to be focused on just the basics:

1.  What is the chemical requested and how much quanitity to be stored?

2.  Is it food safe?

3.  Are there any safety concerns beyond what the label says that might be necessary?

 

Does anyone have a template for a short form like this already?  What other information may need to be included on this form?  We are trying to keep it to one page MAX.

Hi TAW

We have a one page Chemical Request Form (see attached) that specifies what the chemical will be used for and if it replaces a current one in use. Includes safety aspect for PPE and training requirements.  Hope you can find something useful to use in your application!
