Documents signed by employees showing their commitment to Food Safety

Hi,
I visited a company a few years ago where they had a document (other than the Quality Policy) on display in reception that had been signed by all employees showing their commitment to Food Safety and Quality.
Does anyone have an example of this that I can use to develop something similar for my site.
We would also be looking to introduce something similar for Health & Safety
