HACCP What Likelihood - Severity needs a Preventive Control
Good day everyone.
I have two simple (hopefully) questions.
I inherited several HACCP plans that have an inconsistency in the assignment of Preventive Controls (PC) per the Likelihood - Severity matrix.
We use a 5 x 5 likelihood-severity matrix (attached), with 1 being least likely/severe and 25 being most likely/severe.
My predecessor always had a PC for a result of 12 on our Hazard Analysis Risk Assessment chart, but only sometimes for a 10.
Q1. At what number should I always list a PC?
Q2. Are allergens always rated as a "fatality" for severity?

I am not FSMA user but IMEX significant FS hazards (ie requiring direct control) are typically not generated from a "slight[rarely occurs] likelihood". The selection of 10 as requiring direct control is questionable IMO. Which situations did yr predecessor use it ?
Q2. Are allergens always rated as a "fatality" for severity?
I handle allergens within a PRP program so yr query does not apply. Most (labelled) allergens are zero-tolerant but not all. Regardless, all are probably regarded as "High" Severity.
