That's more than just an 'GMP internal audit template,' that sounds like a whole internal audit policy/procedure.

I need a form for my maintenance department to sign after my monthly GMP inspection that has a target date and somewhere for my to sign off that it has been completed.

Your question comes up a lot, try the search function up top as well and you might find some more examples.

Honestly though, you are going to probably be better off just creating it yourself. If  you aren't in the habit of creating things like this yourself, now's a great time to start. I find I use the skill quite a bit.
