Risk Assessment Template and Supplier Recall Database

Hi!

I will be responsible for developing the supplier approval program at the company that I work for. Does someone have a supplier approval risk matrix template that can share and explain how this should be done?

Also, I understand that to do this I have to make a risk assessment, and part of doing a risk assessment is accessing to historic recall data. Does someone know where I could find a good food ingredient recall data base? Most I find are for finished goods.
