What to include on a MSS deviation form?

I'm planning on using a deviation form for when MSS can't get completed. Does anyone have a form they could post or what all would go on a form such as this?

I guess it all depends on how critical the missed item is. (Risk Assessment)

 

As far as a form, keep it simple.

MSS Item Not Completed: XXX      Date/Week Scheduled: XXX     Reason for Non-Completion: XXX  Date Completed: XXX   Completion Verified By: XXX

 

You document that the item was not completed, you give a reason why, and then show when it was actually done.

Is there any reason why you are thinking of going with a specific deviation form for MSS compliance, instead of just utilizing your CAPA program?
