Hello,

Do you perform a root cause analysis for any type of deviations and identify preventive actions?

Having trouble to figure out if we need to to do a RCA for every little deviation.

We apply corrections to fix the situation , but do we need an actual root cause and preventive measures for every deviation?

Thanks

That depends on your workload: if you have time you may do it. However, if you have a lot of "little" deviations (especially repetitive ones) that could mean that your CA/PAs are not effective, and you really need to go deep down to figure out the root causes.

Hi Olenazh,

We perform trend analysis to identify if any of the deviations are reoccurring,

I wasn't sure what is the SQF requirement.

Thanks

Sorry, I'm not SQF, we're with FSSC22000: this code doesn't have extra-strict requirements as other codes do...

Altermanm- If it is a deviation it is a deviation and you need to address it as such. If you can share what you are trending (what types of deviations) it might help to see if its more employee training, the way a program is written, or is it a breakdown at a CCP? Sometimes when writing polices you can write yourself into a corner so it matches the SQF code but is not doable in reality.

ask yourself if the corrective action was sufficient and whether or not this was a deviation to a CCP

No CCP, not all will require further investigation

My deviation form allows us to categorize the corrective actions fpr a deviation as "justified"   "n/a"  or "corrective actions"

So let's say that production uses the incorrect label but catches it on the 3rd package and stops production, changes labels and verifies they have the right one now.  Do you need to investigate this one off?  I wouldn't

Now fast forward, you're seeing at least 1 of these types per month, now you've got to investigate why it keeps happening, so you'd need root cause and preventative measures, but remember, you need to also verify that those preventative measures have to been observed and measured as effective or not so that you can close out the deviation.............that could be a ton of work for you

Hello there!

We conduct root cause analysis if the deviation is related to the following:

- CCP breakdown

- OPRP breakdown

- Potentially unsafe product (PUP)

- Food Safety Emergency

- Complaints

- Management directive

- Recurring incident

- Audit Findings
