Clause 3.5.2.2 requires a questionnaire for each haulier as part of the approval process. Is this questionnaire for them or us to fill out?

Hello, welcome to the forum. If you, as a storage/distribution company, are using third party transport - you need them to complete a questionnaire to make sure they comply with regulatory, customer and your corporate requirements in terms of food safety. If you're using your own transport, you need to make sure it's meeting those requirements as well.

It is for the hauler to complete and provide supporting evidence that what they say they are doing they actually are vs. Pencil whipping.
